What Do I Bring to My Appointment?
- health insurance card
- a list of your questions and concerns
- a list of all medications you are currently taking
- a copy of your CT scans and MRI, not just the report, but the actual images on CD
- copies of records from other doctors pertaining to your specific problem
- completed forms from our website
